Fill in the information requested below. Once we receive your submission, we will mail the Sponsor form directly to your Sponsor. They will need to fill out the form and have their Pastor sign and seal the document. Your Sponsor will also be sent a return envelope pre-addressed and pre-stamped to return the completed form to SIJ.
If your Sponsor is a parishioner of SIJ, Fr. Brian will sign and seal the form once it is returned to us.
